1. Agreement to terms
These Terms of Service govern your use of the Forma Wellness website and any coaching programmes, sessions, or materials purchased through it. By booking a call, purchasing a programme, or otherwise engaging our services, you agree to be bound by these terms.
If you do not agree with any part of these terms, please do not use this website or purchase our services.
2. Services offered
Forma Wellness provides health and lifestyle coaching services, including but not limited to nutrition guidance, movement planning, sleep and stress coaching, and accountability support, delivered via one-on-one sessions, group programmes, and digital resources.
- Coaching sessions are conducted via video call unless otherwise agreed.
- Programme inclusions vary by package and are described on the Services page.
- Session scheduling is subject to coach availability.
3. Health disclaimer
Important
Forma Wellness coaching is not a substitute for medical advice, diagnosis, or treatment. Our coach is not a licensed physician, dietitian, or mental health professional unless explicitly stated. Always consult a qualified healthcare provider before making changes to your diet, exercise routine, or medication, particularly if you have an existing medical condition.
By engaging our services, you confirm that you are physically able to participate in the recommended lifestyle changes and that you assume full responsibility for any risks associated with doing so.
4. Payments & refunds
Full or partial payment is required to confirm a booking, as specified at checkout. Programme prices are listed in Indian Rupees (INR) and are subject to change without affecting active bookings.
- Monthly programmes are billed in advance and auto-renew unless cancelled.
- Fixed-term programmes (e.g. the 90-Day Reset) are billed as a single upfront payment.
- Refunds are not provided once a coaching session has been delivered, except where required by law.
- Unused sessions are non-transferable after 30 days from purchase.
5. Cancellations & rescheduling
Sessions may be rescheduled with at least 24 hours’ notice at no charge. Cancellations made with less than 24 hours’ notice, or missed sessions without notice, will be counted as used.
Monthly programmes may be cancelled at any time, effective at the end of the current billing cycle.
6. Client responsibilities
You agree to provide accurate health and lifestyle information during onboarding, and to inform your coach of any changes to your medical condition during the course of the programme. Coaching outcomes depend significantly on client adherence and individual factors, and no specific result is guaranteed.
7. Limitation of liability
To the maximum extent permitted by law, Forma Wellness and its coach shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services, including but not limited to any adverse health outcomes resulting from failure to disclose relevant medical information.
8. Privacy
Personal and health information shared during coaching is kept confidential and is not shared with third parties except where required by law or with your explicit consent. For full details, see our separate Privacy Policy.
9. Changes to these terms
We may update these terms from time to time. Continued use of our services after changes are posted constitutes acceptance of the revised terms. Material changes will be communicated to active clients directly.
